Waiver & Acknowledgements:

USA Wrestling Requirements: Acknowledge that all listed wrestlers in this registration will be registered for a USA Wrestling Athlete Card prior to participating in any activities(including practice). If pre-arrangements are needed please contact using our contact form, to obtain necessary documentation needed for upload as part of the Bulldog registration.

Payment: $150.00 per family to include Folk style, Freestyle, and Greco Sessions for the season. This registration is non-refundable.

Medical Consent: In the event of injury, I authorize the program officials of Nampa Bulldog Wrestling Club to arrange for such medical services as may be deemed reasonable and necessary to the welfare of injured party. I release the Nampa Bulldog Wrestling Club and all others from liability in taking such action, including all action, which may be contrary to my personal religious beliefs.

Liability Release: I agree to release the Nampa Bulldog Wrestling Club and all other cooperation’s, agencies, employees, coaches, officials and or managers thereof from all liability for damages by reason of injuries or property damages that may be sustained as a result of participating in this program. I have read this release and consent to medical treatment and understand all of its items. I sign this voluntarily with the full knowledge of its significance.

Electronic Signatures: Each party agrees that this Agreement and any other documents to be delivered in connection herewith may be electronically signed, and that any electronic signatures appearing on this Agreement or such other documents are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility
